Philosophy
Persons with Alzheimer’s disease and related dementias can be assisted in maintaining their dignity, independence and sense of purpose and well-being in a supervised, structured and loving environment. Their abilities should be acknowledged and they should be treated with respect, offered meaningful activities and encouraged to function as independently as possible. A person-centered, wellness philosophy will be utilized throughout the program. This model puts the resident before the institution and nurtures the health and wellbeing of the whole person – mind, body and spirit.
Mission
The mission of Congregational Home’s Memory Care program is to enrich the lives of persons with Alzheimer’s disease and other forms of dementia, to support their family members and caregivers with compassion and to be a role model for other facilities.
Program
The Memory Care program at Congregational Home provides a person-centered approach with a strength-based care plan.
Within Congregational Home you will find two separate Memory Care units. Memory Care Unit I (MCUI) was developed for individuals who are in the mid-stage of Alzheimer’s disease or a related dementia. Memory Care Unit II (MCUII) is for individuals whose disease process is more advanced.
MCUI accommodates up to 12 residents in a small household setting. Within that household, based on resident abilities, the program offers a structured day which has proven to increase comfort and reduce anxiety. Flexibility and spontaneity is promoted in that environment. This wellness model encourages positive social interactions, activity-focused care and fosters independence with activities of daily living.
MCUII, a secured setting, can accommodate up to 17 residents. The program focus on this unit often requires staff to anticipate residents’ special needs, such as food preferences and sleep patterns. The specially trained staff concentrates on maintaining a structured environment that promotes a balanced level of stimulation and rest. The focal point is on one-to-one staff to resident visits and individual projects.
